To be eligible for health care:

  1. The patient must reside or work in Coffee, Franklin or Moore Counties.
  2. The Patient and/or at least one member of the household must be working and is not eligible for TennCare and is not covered by any other medical insurance.
  3. The patient must fall within the income guidelines.

New patients should call 931-455-5014 to verify eligibility.

Required Documentation for approval:

  • Two most recent pay stubs
  • Utility or phone bill for residence verification
  • Last year's income tax return.
